LMC's social workers and discharger planners are available to serve patients and families at the hospital, at the Dialysis units in Rice Lake and Spooner, and through the Home Health Care and Hospice programs.

We know illness can affect you and your familyphyscially, emotionally, socially, and financially.

At your request, or your doctor's, a social worker or discharge planner can assist with:

• counseling
• referrals
• arrangements for community services
