Position Description
The Social Worker provides psychosocial evaluation, counseling and resource referral services to patients and as needed with families, works in collaboration with the multidisciplinary treatment team to incorporate psychosocial needs and concerns in planning care.  This position can be an advocate to the development of resources to meet patient needs.  This position participates in patient, family, and staff orientation, education and in-service activities.  This position will work with the LMC Home Health Care / Hospice.

Qualifications
Must have a Bachelor’s or a Master’s in Social Work and State of Wisconsin license or certification for the position supporting Home Health Care / Hospice.  Minimum of two years of clinical Social Work experience, preferably in a medical setting.  Experience preferred in hospice.  Must have a valid driver’s license and be able to provide own transportation with mileage to be reimbursed.

Normal Work Schedule
Hours – 8am to 4:30pm – Monday thru Friday – variable days.
