Social Services
LMC's social workers and discharge planner are available to serve patients and families at the hospital, at the Dialysis units in Rice Lake and Spooner, and through the Home Health Care and Hospice programs.
We know illness can affect you and your family physically, emotionally, socially, and financially.
At your request, or your doctor's, a social worker or discharge planner can assist with:
- counseling
- referrals
- arrangements for community services
