Planning For Discharge
The discharge planning process is focused on the continuing care needs of the patient and the family. This process combines the expert knowledge and experience of your entire care team who will help you and your family identify health and wellness needs after your hospital stay. When your physician determines that you no longer need specialized inpatient hospital services, he or she will issue discharge instructions and orders. Your nurse will review the instructions with you so you understand your continuing care needs. Staff will assist you to your vehicle or assist you with making arrangements for a ride home.
Social Workers are available at no charge to refer yout o community resources and to help arrange for services you and your family may need. They can connect you with resources in home healthcare, therapy, medical equipment, home-delivered meals, support groups, rehabilitation programs, nursing home or alterative living arrangements. Home Care & Hospice Services
Your care team will help you look at all care options available and arrange for assistance that you may need such as supportive help, home care nursing, therapy or hospice care.
Lakeview Medical Center also performs a wide variety of rehabilitation services both as inpatient and outpatient. Services provided include: Occupational, Physical, Respiratory, Speech and Language therapy.
